Federally Qualified Health Centers (FQHCs) serve a disproportionately high share of Medicaid patients in New York — including a significant number of pregnant and postpartum women. This makes FQHCs uniquely positioned to connect patients with the HRSN meal benefit, one of the most impactful and least-utilized Medicaid benefits available in 2025.
What Is the HRSN Meal Benefit?
Under New York's NYHER 1115 Medicaid Waiver, Medicaid-enrolled pregnant and postpartum women are entitled to 21 free home-delivered meals per week for up to 15 months. The program is fully funded by Medicaid — zero cost to patients or providers.
Why FQHCs Are Ideal Partners
- High concentration of Medicaid-enrolled postpartum patients
- Existing SDOH screening workflows (PRAPARE, AHC) that identify food insecurity
- Trusted relationships with patients who act on clinical recommendations
- Alignment with value-based care and social determinants goals
How the Referral Works
Orivia Care's referral model requires minimal workflow change. Your team identifies an eligible patient, gets verbal consent, and shares a referral link. That's the entire referral. Orivia Care and United Hands handle screening, enrollment, and all ongoing coordination.
Integration With SDOH Screening
If your FQHC uses PRAPARE or similar SDOH screening tools, HRSN meal referrals integrate naturally into your existing workflow. A positive food insecurity screen is a direct trigger for an HRSN referral. No new systems, no new training required.
